A lot of people have asked me this question, and it’s really not that hard.
A Sole-Proprietorship, or a self-owned business, doesn’t have a dedicated tax-id number.

Option 1:
On the application form, just type in “Chaim’s tutoring services” as the business name, and leave the Tax ID field blank. Most applications, including Amex say by that field “If left blank, we will assume you are a Sole Proprietorship”

Option 2:
Repeat your social security number in the tax-id field, as they are one and the same for self-owned businesses.

I’d like to hear back from people who apply with these methods and still get rejected.

i applied for the starwood business three weeks ago when you originally posted the deal, i wrote down a company name using my social for the tax-id, i was contacted and told that i had to send in proof of my company, e.g. bank statements, attorneys letter etc.. then i told them that i was sole propietorship, and they said “oh thats o.k., a lot of people don’t realize what to write, since there is not an option for it on the application page”, and they went ahead and approved me right away.
