you will receive credit notification via email. I got mine instantly. It said:
Dear xxxxxxxx
Thank you for downloading an app from the Amazon Appstore. We have added a $5.00 promotional credit to your account. We will automatically apply this credit to future purchases of any apps or in-app items.
Thanks. Confirmation of your $5 pops up as soon as you click on download of any free app that you choose in the amazon app store

Just bought an app with the credit and got another email that my purchase entitled me to a $1 credit in the MP3 store. The email pointed me to http://www.amazon.com/gp/feature.html/?&docId=1001927001
